2.     Call 897-0816 once you decide you would like to take the test. We will set up an appointment for you to come in to our office sign our consent form and make your payment of $20

3.     Go directly to the participating laboratory and have your blood sample taken.

4.     Call us for your test results 10 business days after your test was done, your councilor will explain this is step in more detail.
